Request for Quotation for Supply and Delivery of Training Supplies

The Agricultural Training Institute invites interested suppliers to submit quotations/proposals for the supply and delivery of training supplies and materials for the two batches of Training on Basic Monitoring and Evaluation for RDE Stakeholders on May 7-11 and July 9-13, 2018.

The approved budget for the contract is Php 83,252.50.

Instructions:
1. Suppliers are required to read the instructions and fill all the blanks properly. The official Quotation Form must be used in accomplishing their bid proposals / quotations.
2. Any specifications other than those required / stated in the form shall not be considered in the evaluation of bid.
3. Quotation/s must include all kinds of taxes for the item listed hereunder, including delivery charges.
4. Always indicate the brand name / model of the offered item(s) or product.
5. Price quotation(s) submitted shall be valid THIRTY (30) DAYS from the deadline for the submission of quotations.
6. Terms of payment - within THIRTY (30) days from the delivery of items.
7. Quotations/proposals shall be submitted to the BAC Secretariat's office, Supply and Property Management Office, ATI Central Office, Elliptical Road, Diliman, Quezon City.
8. Only sealed canvasses shall be accepted.
9. The bidder is required to affix his/her signature across the flap of the sealed envelope.
10. DEADLINE FOR THE SUBMISSION OF SEALED QUOTATIONS: APRIL 10, 2018, 9:00 AM.
11. Validity of stocks: 30 Days
12. Awarding shall be done by lot.
